2. To Enter: Complete the entry form at www.bergenhealthandlife.com. Your entry must be received no later than August 1, 2008. No correspondence will be answered. Bergen Health & Life will not be responsible for lost, late, misdirected, incomplete, incorrect or otherwise undeliverable entries.
3. Prizes: On or about August 1, 2008, a random drawing from all eligible entries will be conducted. All decisions are final. The following prize(s) will be awarded: one copy of the book The Brief Wondrous Life of Oscar Wao, signed by the author. Odds of winning depend on the number of eligible entries received. Winner will be notified by phone. Prize(s) must be claimed no later than August 29, 2008. Prizes not claimed within such time will be given to an alternate winner. Prize(s) are not transferable and may not be given, bartered or sold. There are no cash prizes or prize substitutes. Winners are responsible for all local, state and federal income taxes associated with winning the prize(s).
4. Limitations of Liability: Entrants agree to be bound by these rules and that the decision regarding the winner is final.
